Preparing for Provisional Certification Site Visit

  1. You will be notified via email by the State Affiliate and asked to work out a date for the visit.
  2. Once you have an appointment for a visit, you will receive an email from PLTW Ohio confirming the appointment. Attached to this email, you will find the Ohio Cover Sheet; please complete it and email it back to deb.lukjanovs@sinclair.edu
  3. Go to http://www.pltw.org/Assessment-Evaluation/School-Certification.cfm and read the Certification Documents Guide, the Guide for Schools and the other documents concerning certification.
  4. Complete and submit the self assessment. (Certification Document 2)
  5. Print out a copy of the Site Visit Guide (Certification Document 8) both to use as a rubric and to assemble documents you will need to show the visiting team. We will use the Guide to formulate our questions and ask to see the documents listed below. This is a PROVISIONAL visit, so you may not yet have available all the documents called for. Please provide all that you have.

Documents:

  • Additional copies of cover sheet and self assessment
  • Teacher Certificate(s) of Completion for courses taught
  • Counselor Certificate of Completion for counselor conference
  • List of Advisory Council members
  • Minutes of last 2 Advisory Council meetings (if available)
  • List of any internships offered
  • Ten randomly picked PLTW student schedules
  • At least three student portfolios
  • Pathway showing course implementation and how courses are scheduled
  • Copies of any local articulation agreement(s) with post-secondary institutions
